Mobile Home Care Staff Nurses Needed

Job Description:

Patient assessment: Conduct comprehensive assessments of patients' health status, including medical history, vital signs, physical examinations, and functional abilities.
Care plan development: Create individualized care plans based on patient needs, including goals, interventions, and medication management.
Medication administration: Accurately administer prescribed medications according to doctor's orders, including monitoring for side effects.
Wound care: Manage and care for wounds, including cleaning, dressing changes, and monitoring for infection.
Vital sign monitoring: Regularly monitor vital signs like blood pressure, heart rate, and temperature.
Personal care assistance: Assist patients with activities of daily living such as bathing, dressing, eating, and toileting as needed.
Patient education:Provide health education to patients and their families regarding their conditions, medications, and self-care practices.
Communication and collaboration: Maintain open communication with patients, families, physicians, and other healthcare team members to ensure coordinated care.
Documentation: Accurately document all patient care activities, assessments, medications administered, and any changes in patient condition.
9am to 6pm daily with possibility of weekend on call.
